Pair of Marlon Brando signed physical documents for insurance purposes regarding his role in the 1992 film "Christopher Columbus: The Discovery"
First document is dated November 15, 1991 with ten routine health-related questions for the actor to answer, such as any heart, breathing, stomach, etc. issues recently.  The examining doctor put his notes on the right side pertaining to the questions Brando answered "Yes" to.  Below the yes and no questions, #5 asks if he or she had been disabled in the past three years while filming another project.  Notation of "Prostration while shooting The Freshman, 1990".
This document is the first one in my photos above and measures 8.5 x 9".
Nicely signed by Brando in blue ball-point at its completion.
Second document bearing the legendary actor's signature measures 8.5 x 5" and appears to be the doctor's general findings which would be sent to the company providing insurance on the film.
Eye, ear, nose and throat; heart and lungs, all "NAD"......"no abnormality detected"
Again, nicely signed by Brando at the bottom, just above the physician's.
Both of these rare documents are in Excellent to Near Mint condition.  Very well-preserved.
